In our world of person-centric care, medication management needs to flow seamlessly and accurately from practitioner to practitioner and from location to location. Unfortunately, we all know that it doesn’t often happen that way. Records from various facilities merge incorrectly, medication lists don’t always get updated as patients’ diagnoses and daily routines change, and patients inaccurately remember their own prescriptions when they describe them to those providing them with care.
That’s why at GPM’s 2019 LTC Summit, John F. Derr, RPh, FASCP highlighted how it is our responsibility as practitioners and IDT members to improve upon these processes – and now. He delved into the critical role that medication management plays within our spectrum of care to improve health incomes and how it’s a primary place to focus on when it comes to the interoperability of electronic medical record technology.
